 II. SERVICE REQUEST

 
 

STEP 1

Receiving/Recording

CSO identifies the service type, prepares the Job Order (JO), and keeps a record

STEP 2

Job Order Appoval

  • Division Manager and General Manager approves the service request
  • Plumber Assignment

STEP 3

Repair

  • Plumber retrieves the materials from the Storekeeper A to conduct the repair
  • Consumer to sign the Job Order with the completion date, remarks, and service rating
  • Plumber returns the JO to CSO for filing and inspection

 

The Maramag Water District Frontline Services receives the complaints from the walk-in concessionaires and complaints filed through the Hotline Nos. 09679502070, 09164551126, & 09982875089. The Administration and General Services Division assist the Frontline Services in receiving the complaints/inquiries/service requests through a social media platform: (Facebook Official Page: facebook.com/waterdistrictMaramag)

 

 III. NEW CONNECTION

Step 1: Customer Service Officer (CSO) receives the requests, responds to the inquiries, hands out the Application form to the concessionaire.
Step 2: Applicants are required to attend the orientation seminar every Monday at 1:30 PM to 2:30 PM. The discussion pertains to the Policy Contract Agreement and concessionaire’s role in the conservation and preservation of water supply, securing water connection, & among other rules and regulations that protects the welfare of the consumers and the Water District interest as well.
Step 3: Requirements validation. Please see the list here for Residential, Government, Industrial, and Commercial connections
Step 4: CSO reviews the completed application form and issues payment slip
Step 5: Payment processing and contract signing
Step 6: General Manager signs the Application form and Contract Agreement
Step 7: Applicants to process the notarization of the documents on Step 6
Step 8: CSO to issue a Job Order upon the submission of the notarized documents
Step 9: Installation endorsement to the plumber

  • Withdrawal & releasing of water meter to the plumber
  • Installation of the water meter
  • Inspection of the newly installed water service lines
